The first call asks more than a number

CDC describes the US quitline intake as asking about current tobacco use, prior attempts, how you heard about the service and demographics. Counselling then discusses the caller’s situation. This is an outline of US quitlines, not a questionnaire every clinic in the world uses.

These questions are not interchangeable. Tobacco-use details describe the starting point; an earlier attempt gives you something concrete to discuss. ‘How did you hear about us?’ is not a measure of your motivation, and giving a phone number does not itself complete a counselling assessment.

An earlier attempt: describe what happened, not a score

You can separate the method you tried, what you found difficult and what you want to discuss now. For example: ‘I stopped for a while last year; I don’t remember the dates, but work breaks were difficult.’ This invented reply contains usable context without inventing a timeline.

If an online form insists on a date you cannot remember, ask whether the service accepts an estimate, an unknown answer or clarification by phone. Do not turn a guess into an exact-looking date. You also need not add colleagues’ names to explain a work-break situation.

Health information has a different recipient and purpose

If the conversation moves into medical care, tell the qualified professional about relevant symptoms, medicines and medical history by the agreed route. A past medicine name and a symptom you experienced are information to discuss, not a basis for choosing a new medicine here.

A short note can help you remember your question or the medicine name. It need not be uploaded as a full smoking diary. Ask a provider which information it needs for that conversation; do not withhold clinically relevant facts merely because they were not on a booking form.
