Hospital abstinence and a home plan are different facts

A smoke-free stay may reflect hospital rules, illness, treatment or a personal choice. It does not prove that the home routine or discharge medicine plan is settled. NICE’s UK guidance includes continuity of cessation support after discharge or referral to local support; it is a service principle, not a schedule for your recovery.

Smoking can stop abruptly in hospital. SPS explains that this change—and a return to smoking—can matter for some medicines, including when nicotine products continue. Tell the discharge or prescribing team what actually happened and whether smoking changed again at home. The page cannot decide whether a hospital medicine should be resumed, withheld or changed.

Separate three unfinished tasks

Home routine: choose one cue you can change without conflicting with recovery instructions, such as moving tobacco out of the place where you usually sit. Clinical instruction: ask the discharge or prescribing team to reconcile a conflicting or unclear instruction. Support referral: check that the receiving service actually has the referral and who will contact you. Completing one task does not complete the other two.

A referral sent is not yet a consultation received

In a fictional example, the discharge record says “referred for cessation support”, but no service has made contact. The next action is to contact the named team and ask whether the referral arrived, who owns the reply and what the backup route is. It is not to recreate a hospital treatment plan from an old web article.

Keep the discharge instructions and private questions together for the real care conversation. New clinical concerns follow the team’s urgent instructions or current local care; immediate danger uses emergency help. Returning home is a transition to organise, not a test of willpower or permission to make medicine decisions.
